Purpose of Study

Heart disease, which includes heart attack, stroke, heart failure, are more common in Black communities. High blood pressure, high blood sugar, obesity, high cholesterol, and smoking increase the chance of getting heart disease. In ACHIEVE-GreatER, we want to find ways to make sure African Americans are empowered to get the best care for their heart and lower their risk for future heart problems. Heart disease is the leading cause of death in African Americans living in Cleveland. You have the chance to make a difference in your health and the health of your community.

Participants will be asked to take a coronary artery calcium (CAC) scoring exam and will be expected to complete 10 total surveys taking approximately 60 minutes of time after the CAC exam, and will have a 6 and 12 month follow-up.

Participants will work with a community health worker to help achieve their health goals. They will also be given a smart activity watch, scale, BP cuff, and air/noise quality monitor to watch their progress.

Who Can Participate?

Self-identified African American males between the ages of 40 - 75 years old who reside in neighborhoods with low socioeconomic status may be eligible for this study.
