NEW: View your health care provider’s notes from office visits in your UH Personal Health Record

To help you take a more active role in managing your health and wellness, we’ve expanded the information now available in your UH Personal Health Record (PHR). As of October 2, 2017, you can view the notes your provider writes during your office visits.

  • Review a summary of your office visit, your treatment plan and next steps.
  • Your signed provider’s note* will be available to you anytime, anywhere from your PC, laptop or mobile device (with the FollowMyHealth app).
  • It’s easy, convenient and free.

Your UH Personal Health Record is not the same as your electronic medical record. The electronic medical record has your full medical history going back many years. Your PHR will not contain your entire medical history as it appears in the UH electronic medical record (EMR).

The chart below shows what information you can view in your MyUHCare PHR.

Can I Add Information To My UH Personal Health Record?

You decide what details will be seen in your UH Personal Health Record. Things in your record should help you and your health care team better handle your care. Below are some basic items you may choose to add to your personal health record.

  • Your primary care doctor's name and phone number
  • Allergies to medicine, food and other things
  • A list of your medicines as well as how much you take, and how many times a day you take them
  • On-going health problems, such as high blood pressure
  • Major surgeries and respective dates
  • Other information that can be helpful to have in your personal health record:
    • Results of screening tests
    • Cholesterol level and blood pressure
    • Exercise and eating habits
    • Health goals, such as stopping smoking or losing weight

Your Medication List from UH Hospitals and UH Physician Offices: Why Are There 2 Sections?

Your UH Personal Health Record (PHR) may contain medical information from UH hospitals and UH physician offices if you have received services as an inpatient or emergency department patient. It also may contain some medical information from UH physician offices that use the UH electronic medical record.

If you were prescribed medications as part of these services, this information will be available in your PHR and will be displayed as “active.” Active means the medications were current or effective at the time of your discharge or visit.

Please refer to the grid below regarding specific dates of service available in your PHR from UH Hospitals and UH Physician’s Offices electronic medical records.

You can locate the Medications section of your PHR by selecting the down arrow next to the My Health tab and then clicking Medications from the menu.

MyUHCare Medications

The Medications tab will list the medications that were prescribed to you at the time of your service.

Medications prescribed from UH Hospital Services may or may not match the medications from physician offices due to changes made by your UH physician as part of your care before or after your discharge from UH inpatient or emergency department visit.

The medications listed in the Hospital Services section will match the summary provided to you upon discharge. You may review the summary and compare it to the list in your PHR.
